Yes, you can! Simply add the photo, along with a space at the end of it. This results in a photo description with no other description (only the title) as shown below.

If itIf it doesn't appear when you type the # key, it may be because you are publishing it as a Public Page. Because it's a Public Page, it can't be accessed the same way as an Internal Workflow. Make sure that it is a Workflow that targets Users and it will appear without any problems.

Why does a field disappear when another field is checked?

If you have a field that disappears when another field is checked or selected, one thing you can do is confirm that the Key is different for both of them. If they have the same key, they are linked and selecting one can affect the other(s) that shares the same Key.
